Plan options

Select the dental coverage that works best for your needs.

Aetna DMO plan

  • Choose a network general dentist to manage all your dental health care and refer you to any network specialists 1
  • Know the exact dollar amount you'll have to pay based on a charge schedule
  • No annual dollar maximums
  • No deductibles to meet

  • The DMO plan isn’t available in these states: Alaska, Ala., Ark., La., Maine, Miss., Mont., N.D., N.H., Puerto Rico, S.C., S.D., Wyo., and Vt.
    • If you live in one of these states, your only option is the PPO dental plan.
  • The DMO plan is available in these states: Ariz., Calif., Ga., Mass., Md., Mo., N.C., N.J., Texas.
    • You must either live or work in a DMO service area in one of these states to choose the DMO plan.
    • You can’t choose the plan if you live in a neighboring state where the DMO plan isn’t available but work in a state where the DMO plan is offered, due to state regulations.

Aetna PPO plan

  • Visit any licensed dentist or specialist (without referrals)
  • Preventive services covered at 100%
  • Calculate your own out-of-pocket costs based on percentages
  • Annual plan maximums apply
  • Individual and family deductibles
  • Plan will cover eligible dental expenses after you meet your deductible

Premiums are deducted pre-tax semi-monthly, except for premiums for domestic partners and their children, which are deducted after tax.
