Preventive care services

Most preventive services are provided at no cost to patients. These include:

• Annual physicals/well-woman exams
• LifeForce labs/lab only visits
• Blood pressure checks
• Nurse visits
• Nutritional counseling (where available)
• Behavioral health (where available)
• Chronic disease management
• Allergy injections
• Flu shots
• Other immunizations
• Non-sick phone visit
• Emergency visits
• Body composition analysis

• $0 copay for teammates enrolled in a Truist PPO, ACO, or Upfront Advantage medical plan, including behavioral health services where available
• $0 copay for nutrition (select locations) for all teammates regardless of medical plan coverage
  
• $35 copay for behavioral health services (select locations) for teammates on a Truist high-deductible plan or a non-Truist medical plan
• $0-$20 copay for preventive/maintenance prescriptions and over-the-counter medications for all teammates regardless of medical plan coverage

Non-preventive services

Non-preventive services will incur a charge based on upon your insurance plan. These include:

• Provider office visits
• Acute care/sick visit by phone or video or in person
• Minor procedures, such as nebulizer treatment, ear lavage, and suturing and removal

• $10 copay for medically covered teammates on the PPO plans
• $35 copay for medically covered teammates on the $1,500 Upfront Advantage PPO plan, a high-deductible plan, or ACO plan and non-medically covered teammates, including Kaiser members
• $10 copay for 30-day supply, $20 for 90-day supply of non-preventive/maintenance prescriptions
