Progression through LifeForce

As you work toward the realistic and attainable goals established with the Peak Health nurse, you can advance to new phases and earn higher medical credits. The program advocates individual accountability for healthy lifestyle behaviors and rewards you for your efforts.

To determine the phase in which you will be placed, the Peak Health nurse will assess your health through a series of tests and compare your results to the program's medical standards. These tests include lab work results, body composition, blood pressure, cardio respiratory fitness, and resting heart rate. 

The nurse also takes into consideration such factors as tobacco use, status and review of labs and age/gender exams, and progress toward goals, including follow-up with your medical provider if it's  outlined as a goal. 

Your phase is influenced by the highest modifiable risk in your situation. The good news is that as participants act on the attainable goals established with their nurse, they may move to a higher phase.

Phase   Minimum Criteria and Next Steps
Phase N You're not a LifeForce participant.
Phase 0 You've completed the Health Assessment, plus lab work and your initial appointment has been scheduled.
Phase 1
  • Completed the Health Assessment, plus lab work and had your first visit with a Peak Health nurse

You remain at Phase 1 if any of the following apply:

  • Body composition is 6.0 points above the target for your age and gender
  • Waist circumference is 8.0 inches or above the target for your gender
  • Fitness level is low
  • Not being seen for medical follow-up by your health care provider
  • Labs and age/gender health exams are over six months past due

Next step

  • Appointments are scheduled every four months.
Phase 2

Minimum Criteria (all criteria must be met or you’ve made progress towards goals; otherwise, your phase will be lower)

  • Body composition is between 4.1 and 6.0 percentage points above the target for your age and gender
  • Waist circumference is between 4.0 and 7.9 inches above the target for your gender
  • Your fitness level is below average
  • You're being seen for medical follow-up by your health care provider
  • You may be using tobacco and/or nicotine products
  • All age and gender health exams and labs are current within six months
  • Your health assessment is current (If your spouse or domestic partner is covered under a Truist medical plan, their health assessment also must be current)

Next Steps

  • You're entitled to receive a medical credit on your paycheck starting the next month
  • Appointments are scheduled every four months
Phase 3

Minimum Criteria (all criteria must be met or your phase will be lower)

  • Body composition is between 2.1 and 4.0 percentage points above the target for your age and gender
  • Waist circumference is at or 3.9 inches above the target for your gender
  • Your fitness level is average 
  • You're being seen for medical follow-up by your health care provider
  • You've stopped using tobacco products for more than three months, or you're currently using tobacco or nicotine and are either enrolled in a cessation program and/or are receiving medical assistance to stop using tobacco/nicotine products
  • All age and gender health exams and labs are current within three and six months
  • Your health assessment is current (If your spouse or domestic partner is covered under a Truist medical plan, their health assessment also must be current)

Next Steps

  • You're entitled to receive a medical credit on your paycheck starting the next month
  • Appointments are scheduled every four months
Phase 4

Minimum Criteria (all criteria must be met or your phase will be lower)

  • Body composition is within 2.0 points of target for your age and gender
  • Waist circumference between 0.1 and 4.0 inches below the target for your gender
  • Your fitness level is good to excellent
  • All modifiable health risks have been addressed
  • You've stopped using tobacco or nicotine products for more than three months and you're not enrolled in any smoking cessation program or receiving any medical assistance to stop smoking
  • All age and gender health exams and labs are up to date by your appointment
  • Your health assessment is current (If your spouse or domestic partner is covered under a Truist medical plan, their health assessment also must be current)

Next Steps

  • You're entitled to receive a medical credit on your paycheck starting the next month
  • Appointments are scheduled every eight months
Phase 5                    

Minimum Criteria (all criteria must be met or your phase will be lower)

  • Body composition is at target for age and gender
  • Waist circumference at least 4.1 inches below the target for your gender
  • All modifiable health risks have been addressed and are being maintained (all goals are considered maintenance)
  • You don’t use tobacco or nicotine, you've stopped using tobacco or nicotine for more than 12 months, or you've completed a smoking cessation program and a health care provider has certified you’re medically unable to stop smoking at this time
  • All age and gender health exams and labs are up to date by your appointment
  • Your health assessment is current (If your spouse or domestic partner is covered under a Truist medical plan, their health assessment also must be current)

Next Steps

  • You're entitled to receive a medical credit on your paycheck starting the next month
  • Appointments are scheduled every 12 monthss

Remember, manageable short- and long-term goals are established with the help of the Peak Health nurse. You must continue to make progress toward your goals, which includes maintaining regular evaluations, to maintain an active status in the LifeForce Program and be eligible for a medical credit.

If you're unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn a medical credit, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting a nurse at Peak Health at 252-237-5090. You also may request an alternative standard from your nurse during your appointment.

If your appointment is by phone, you're required to provide your biometrics during your call with the nurse, or you'll be dropped to Phase 1 and lose any medical credit that you'd qualify for otherwise.
